New Federal Law – What You Need to Know
What is changing?
A new federal law, the Paperwork Burden Reduction Act (PBRA), allows large employers to use an Alternative Manner of Furnishing Form 1095-C.
Under this approach, eligible employees will not automatically receive a paper copy of Form 1095-C. Instead, employees may request a copy at no cost if they need one for their records or tax filing.
What is Form 1095-C?
Form 1095-C provides information about the health coverage offered to you during the prior calendar year. You may need this form when preparing your personal tax return or if requested by a tax professional.
How to request your Form 1095-C
You may request a copy of your Form 1095-C at any time using one of the methods below:

Please include your full name, employee ID, current mailing address, and the tax year requested to help us process your request quickly.
When will you receive your form?
Once your request is received, your Form 1095-C will be provided within 30 days, as required by federal law.
How long will this notice be available?
This notice will remain posted and accessible through October 15 of the year following the applicable tax year, in accordance with IRS requirements.
